In 2003 my husband (who is a AmE native speaker) came to Italy for 9 month with an exchange program and he became really quite fluent.
After that he went back to the US, but came back here after two years and in one year he achieved amazingly good results. I think now most people don't even notice that he's not a native unless they talk to him for a long time (because every now and then he says something vaguely awkward).. right now he's in the living room discussing politics in Italian with my mom :D.
So here is my suggestion, come to Italy and speak! Also.. he says that it helped him a lot reading books in Italian that he had already read in English.
References :
personal experience

The best method for learning languages is immersion. So here is a good excuse for taking a long trip to Italy! If you cannot do that, find a Berlitz home course and advertise for an Italian speaker to meet with to practice. Craig's list or Yahoo Answers might help you find someone.
